Need an extension? If your grant is ending, and you'd like to request additional grant funding, get the Fiscal year 2012
instructions for requesting a grant extension. You'll need to complete a form HUD-91186-A,
Multifamily Housing Service Coordinator One-Year Budget as part of your extension request. Send your request to your local Field Office Project Manager or other representative.
Line of Credit Control System (LOCCS)
Housing owners obtain their grant funds from LOCCS. The Chief Financial Officer provides the LOCCS Acces Guidelines for Grantees.
Federal Financial Reports
Grantees must submit a semi-annual Federal Financial Report, SF 425. This report shows the amount of grant funds used over each six month period, and the relationship of the program's expenditures to the total grant amount received. The two reporting periods are October 1 through March 31 and April 1 to September 30. Grantees submit these reports to their Field office representatives no later than 30 days following the end of each reporting period.
Performance Reports
Grant recipients must submit a Semi-Annual Performance Report, form HUD-92456. All Service Coordinators in HUD assisted multifamily housing, whose positions are paid for by grant funds, Section 8 or PRAC operating funds, residual receipts, or excess income, must submit this report. For additional information see our Tracking Performance page.
