Categorically Excluded Not Subject to 58.5 (CENST) format
Use this worksheet for project activities that are Exempt per 24 CFR 58.34 or Categorically Excluded Not Subject to 58.5 per 24 CFR 58.35(b).
Categorically Excluded Subject to 58.5 (CEST) format
Use this worksheet only for project activities that are Categorically Excluded per 24 C.F.R. Section 58.35(a). Must use the Level of Review format in conjunction with the Statutory Worksheet to document compliance with the laws and statutes listed at 24 CFR §58.6.
Environmental Review for Leasing Projects – CEST format
Use the CEST format to complete the environmental review for leasing activities. Leasing on behalf of tenants without development/rehabilitation is a CEST activity subject to the Federal laws and authorities listed in 24 CFR 58.5 & 58.6. HUD grantees may not enter into lease contracts for specific dwelling units until the responsible entity has completed this environmental review and approved, in writing, the specific subject site.
These requirements do not apply to tenant-based rental assistance or short-term payments for rent per 24 CFR 58.35(b)(1) & (2). Use the CENST format to complete the environmental review requirements for TBRA and short term assistance.
Do not use this review form for acquisition, construction or rehabilitation activities.
Rehabilitation activities as defined in 24 C.F.R. 58.35 (a)(3)(i) & 58.35(a)(5).
Environmental Review for Leasing Projects - Continuum of Care
Continuum of Care (CoC) leasing activities on behalf of tenants without development/rehabilitation is a CEST activity subject to the Federal laws and authorities listed in 24 CFR 58.5 & 58.6 and listed on the Limited Scope Environmental Review format. Use the Limited Scope CEST format for leasing activities funded through the CoC program.
The Rehabilitation Environmental Review (RER) and Appendix A formats are a tiered review option for multiple, recurring residential rehabilitation activities in different locations over the period of the Consolidated Plan (but not to exceed a five year period). The tiered review procedures includes the RER and Appendix A formats (permitted by 24 C.F.R. §58.15) and may only be used for the environmental review of the rehabilitation of existing single-family residential structures,per §58.35(a)(3)(i), with or without the acquisition and/or disposition of the existing structure per §58.35(a)(5). It may also be used for the acquisition/rehabilitation/disposition of individual units in a multi-family structure, but not for the rehabilitation/acquisition/disposition of an entire multi-family building per §58.35(a)(3)(ii), where an environmental review using a Statutory Worksheet may be appropriate. The RER/Appendix A protocol may not be used for projects involving changes in use or new construction activities.
After the RER form is completed the Responsible Entity must follow HUD notice and Request for Release of Funds procedures. 24 C.F.R. 58.43, 58.45 and Subpart H.
An Appendix A format shall be completed when individual project sites are proposed for rehabilitation. The Appendix A documents compliance with factors listed at 24 C.F.R. 58.5 and 58.6 and mitigation measures if required for compliance. Both the RER and Appendix A shall be maintained in the ERR.
Notice of Intent to Request for Release Funds (NOIRROF) for RER Reviews
This template is used after the Responsible Entity (RE) completes the RER format. The NOIRROF template contains the language required to meet the public notice requirement in 24 C.F.R. 58.43. The template contains highlighted sections which must be modified by the RE to identify the RE and describe the particular project reviewed.
Environmental Assessment (EA) Format

Site Specific Field Contamination Checklist
This form may be used by the Responsible Entity to document the site visit and as a guide for determining whether a proposed project site and surrounding area is free of hazardous materials, contamination, toxic chemicals and gases and radioactive substances. This document must be filed in the environmental review record.
Notice of Intent to Request Release of Funds (NOIRROF)
This is the template to use when the project requires that the Responsible Entity (RE) provide public notice of its intent to request release of funds for a HUD funded project. The template contains highlighted sections which must be modified by the RE to fit the particular circumstances of the review.
Combined Finding of No Significant Impact and Intent to Request Release of Funds Notice (FONSIRROF)
This is the template to use when the Responsible Entity (RE) has completed an Environmental Assessment that resulted in a Finding of No Significant Impact on the environment and the RE is simultaneously notifying the public of its intent to request funds be released to undertake the project. The template contains highlighted sections which must be modified by the RE to fit the particular circumstances of the review.
Finding of No Significant Impact
This is the template to use when the RE has completed an Environmental Assessment that results in a Finding of No Significant Impact to the environment and the Notice to the public of the Intent to Request the Release of Funds will be published/disseminated at a later date.
HUD Form 7015.15, Request for Release of Funds and Certification (RROF)
The 7015.15 is submitted to HUD, with a copy of the affidavit of publication, after the comment period has ended. This is the only valid RROF form (exp. 10/31/2014), older versions are invalid.
